BCI Background Checks

All Heidelberg College students must provide documentation of a BCI background Check prior to seeing clients in practicum and internship.  Students may receive a free BCI background check at any of the following locations:

*    OCSW&MFT CT Registration Form

*    Sample Professional Disclosure

(all CTs must have a PDS that is provided to each client seen)

*    Training Supervision Agreement

*   Petition for Practicum Form

*   Lab Evaluation Forms
